Folate, iron and iodine — the core prenatal triad

Folate: the US Public Health Service and CDC recommendation is 400 µg/day folic acid for all women capable of becoming pregnant, with 600 µg/day during pregnancy and 500 µg/day during lactation, to reduce neural tube defect risk. Two forms dominate: synthetic folic acid (pteroylmonoglutamic acid) — the form with the strongest NTD-prevention evidence base, mandated in US grain fortification — and 5-methyltetrahydrofolate (5-MTHF) as calcium or glucosamine salt, marketed on the bioavailability argument particularly for MTHFR-variant populations. Both are legally usable; the regulatory and clinical position is that folic acid retains the strongest population-level NTD evidence and 5-MTHF is acceptable substitution. Upper limits: the IOM tolerable upper intake level (UL) for folate from fortified food and supplements is 1000 µg/day from synthetic forms (no UL applies to natural folate from food). Iron: the prenatal RDA is 27 mg/day. Tolerability is the dominant formulation challenge — ferrous sulphate and ferrous fumarate have the strongest evidence but cause GI side effects driving non-adherence; iron bisglycinate, carbonyl iron and polysaccharide-iron complex offer improved tolerability with documented bioavailability. Iodine: WHO and US guidance is 150 µg/day for adult women, 220 µg/day in pregnancy and 290 µg/day in lactation; the American Thyroid Association recommends prenatal supplements contain 150 µg potassium iodide. The UL for iodine in pregnancy is 1100 µg/day. Surveys repeatedly find a meaningful fraction of US prenatal products mis-specify or omit iodine despite the recommendation.

Vitamin A teratogenicity ceiling and other UL-sensitive nutrients

Other UL-sensitive nutrients in the prenatal window include vitamin D (UL 4000 IU/day), vitamin B6 (UL 100 mg/day — relevant where megadose B6 is marketed for nausea), zinc (UL 40 mg/day), selenium (UL 400 µg/day), copper (UL 10 mg/day) and manganese (UL 11 mg/day). The discipline is per-ingredient specification against the pregnancy UL with formal release and stability assay against the upper bound, not only the label claim lower bound.

Contraindicated and cautioned botanicals in pregnancy and lactation

An extensive evidence base supports contraindication of specific botanicals in pregnancy and lactation. Absolute contraindications include comfrey (pyrrolizidine alkaloid hepatotoxicity), pennyroyal, juniper berry (high dose), blue cohosh (cardiotoxicity to fetus), black cohosh in pregnancy, dong quai, feverfew, goldenseal, kava, and ephedra (banned in US supplements).
